Why Willowbridge exists
Medicare's care-management programs pay practices to do work that genuinely helps patients: coordinating care between visits, managing medications, navigating serious illness, watching the numbers that predict the next hospitalization. The hard part was never the care. It's proving — months later — that the care happened exactly the way the claim says it did.
Most teams track that work across spreadsheets and disconnected notes, and find the gaps only when a payer asks about a claim from March, in November. Willowbridge was built to close that gap. One encounter timer captures the time, the note is drafted from the chart, and every claim is queued with a contemporaneous evidence file: consent on the date of service, the care-plan version pinned to that date, the time log with its timestamps and signers, the threshold-met indicator, modality, and place of service. The audit packet is assembled before anyone asks for it.
What we do
WillowBridge runs the whole non-face-to-face loop on one platform — eligibility and consent, the care plan, time capture, note drafting, claim assembly, and the revenue-cycle workflow behind it — across CCM, PCM, APCM, BHI/CoCM, PIN, CHI, TCM, home-health certification and oversight, RPM, ACP, and the Annual Wellness Visit. Your team works the patient panel; the billing happens around them, with the documentation that defends it.
Where Willowbridge came from
Willowbridge was founded by a physician who had seen the gap from both sides — as a clinician delivering coordinated care, and as the leader of a large multi-specialty medical group where that coordination kept breaking down. The work mattered and patients needed it; there was simply no easy way to perform these services at scale.
To build it right, the founder drew on the knowledge and experience of the people who do this work every day — care managers from large value-based care organizations, and nurses who served as front-line care navigators performing transitional care management. They had all run into the same wall. The manpower it took to track time and stay ahead of every patient's needs made doing this by hand almost impossible — and even when the time did get tracked, actually billing for it brought a quiet sense of dread. Would handwritten notes and a spreadsheet someone in the office maintained really hold up the day a payer asked? The tools that existed weren't built to carry any of it. That hard-won, real-world expertise is reflected in how Willowbridge works.

We don't claim what we haven't earned
SOC 2 Type I is in progress — readiness complete, the independent examination being scheduled. We describe the controls in place today rather than display a badge we don't yet hold.
Your data is yours
Export patient and billing records anytime. On termination you get a 90-day window to extract everything, after which we securely destroy it and certify the destruction.
Evidence over assertion
The discipline we bring to your claims, we apply to our own security: controls are automated and checked in our build pipeline, not just written into a policy.

Willowbridge is early by design. We're onboarding our first practices deliberately, with direct founder involvement in every implementation — so the platform gets shaped around how a real team actually works, and that team gets a direct line to the person building it.
