What it is

Home Health Certification & Care Plan Oversight pays the practitioner for the physician-side work of home health: certifying that a homebound patient needs skilled services under a home-health plan of care, recertifying each 60-day episode, and the month-to-month oversight — reviewing agency reports, adjusting the plan, coordinating with the agency — that never happens face to face.

How the minutes add upTwo rails that pair. Certification: G0180 certifies a patient for home health (initial), G0179 recertifies each 60-day episode. Oversight: G0181 bills when the practitioner spends ≥ 30 minutes in a calendar month overseeing a home-health patient's plan of care (G0182 is the hospice counterpart).

Documentation required every cycle

Each episode / month must show:

  1. Certification or recertification with date and the signed plan of care.
  2. The homebound basis, documented.
  3. Oversight minutes itemized — what was reviewed, adjusted, or coordinated, and when.
  4. Agency communications (reports received, orders returned).
  5. Final practitioner sign-off.
