Home Health Certification & Care Plan Oversight pays the practitioner for the physician-side work of home health: certifying that a homebound patient needs skilled services under a home-health plan of care, recertifying each 60-day episode, and the month-to-month oversight — reviewing agency reports, adjusting the plan, coordinating with the agency — that never happens face to face.

Who qualifies
- Patient is homebound, with the clinical basis documented.
- Patient is under a home-health agency plan of care the billing practitioner establishes and signs.
- The required face-to-face encounter supporting certification is documented.
- G0181 requires ≥ 30 minutes of documented oversight in the calendar month.
- One practitioner bills certification/oversight for the patient.

Documentation required every cycle
Each episode / month must show:
- Certification or recertification with date and the signed plan of care.
- The homebound basis, documented.
- Oversight minutes itemized — what was reviewed, adjusted, or coordinated, and when.
- Agency communications (reports received, orders returned).
- Final practitioner sign-off.
